PASSPORT REQUIREMENTS FOR MINOR - NEW APPLICANTS

In order to facilitate the smooth processing of passport applications (New, Renew & Lost), all applicants are requested to book an online appointment using the link below and fill-out the attached fillable Passport Application Form on the email confirmation for submission during application together with other requirements.
The Philippine Consulate requests that the application be typewritten for easy reference during data encoding.
Please write / reflect the following in the Passport Application Form, if not yet written:
- Complete Address including Suburb/Town/Region and Postal Code;
- Complete Name of Wife / Husband in this format: FIRST NAME MIDDLE NAME LAST NAME (Example: JUAN SANTOS CRUZ);
- Complete Maiden / Single Name of the Mother in this format: FIRST NAME MIDDLE NAME LAST NAME (Example: MARIA DEL ROSARIO SANTOS);

- If newly born in Victoria, South Australia and Tasmania, passport application will be processed only AFTER filing a Report of Birth. Please visit this link: Report of Birth (ROB)
- If Report of Birth was registered one (1) year or more from date of passport application, bring original and photocopy of the Philippine Statistics Authority (PSA) (formerly National Statistics Office of NSO) Report of Birth ( Apply here for PSA Birth certificates )
- Appointment is REQUIRED for filing a Report of Birth;
- Report of Birth application can be done without the presence of the newborn. It can also be done by post/mail;
- Report of Birth Application will be released after three (3) working days;
- Passport appointment can be scheduled on or after the release of the Report of Birth;
- Passport application of a Minor applicant should be accompanied by:
- Either Parents – If parents are married and reflected in the child’s birth certificate
- Mother – If parents are not married
- Authorized Adult Companion – Only upon presentation of a Notarized Special Power of Attorney issued by the parents
- PERSONAL APPEARANCE of minor applicant and either parent or authorized adult companion
- Printed BARCODED APPLICATION FORM, which can be found on the confirmation email after making an appointment;
- ORIGINAL AND PHOTOCOPY of PSA Birth Certificate or Report of Birth (ROB) issued by the Philippine Consulate General in Melbourne
- ORIGINAL AND PHOTOCOPY of PSA Marriage Certificate or Report of Marriage of Parents, if marriage is not reflected in the Birth Certificate of the Minor Applicant
- Photocopy of Passport or photocopy of valid Government-issued ID of either parent (click here for the List of Acceptable IDs for Passport Application)
- Photocopy of school ID (if available)
- Valid Australian Visa – Photocopy;
- PHOTOCOPY of the Additional documents as applicable below;
- Passport Fee: A$108.00 Cash and Debit Card Only
- “Express Post” envelope with Signature on Delivery sticker, which can be obtained at any Australia Post.
- Single Passport Applicant – Medium Sized Envelope
- Family Passport Applicant (Maximum of 4 Passport Applicants) – Large Sized Envelope
SIGNATURE ON DELIVERY STICKER
- If born in the Philippines: Certified True Copy of Local Civil Registrar (LCR) Birth Certificate authenticated by PSA
- If born abroad: Original copy of Report of Birth (ROB) issued by the Philippine Consulate General in Melbourne or first indorsement from Consular Records Division
- Personal Appearance of minor applicant and mother or authorized adult companion
- PSA Birth Certificate or Report of Birth
- Passport or Valid Government issued ID of mother or authorized Adult Companion ( click here for the List of Acceptable IDs for Passport Application )
- If applicant is not accompanied by the mother during the application process, applicant must submit a Notarized Special Power of Attorney executed by the mother designating the minor’s companion to assist in the application process. The Special Power of Attorney must be authenticated by the Philippine Consulate, if executed in Australia
- If applicant is not traveling with Mother, applicant must submit DSWD clearance and Affidavit of Support and Consent executed by the mother. The Affidavit of Support and Consent must be authenticated by the Philippine Consulate if executed in Australia.
- Applicant must submit a Notarized Special Power of Attorney executed by the parent/s designating the minor’s companion to assist in the application process. The Special Power of Attorney must be authenticated by the Philippine Embassy/Consulate if executed abroad
- Passport or Valid Government issued ID of authorized adult companion ( click here for the List of Acceptable IDs for Passport Application)
- Applicant must submit DSWD clearance and Affidavit of Support and Consent executed by the parent/s.
- The Affidavit of Support and Consent must be Consularized by the Philippine Consulate, if executed in Australia
- Passport or Valid Government issued ID of authorized adult companion ( click here for the List of Acceptable IDs for Passport Application )
- Personal Appearance of minor applicant and adult guardian
- PSA Birth Certificate
- Valid Passport or valid government ID* of adult guardian ( click here for the List of Acceptable IDs for Passport Application )
- DSWD Clearance
- Affidavit of Guardianship
- Personal Appearance of minor applicant and adoptive parent/s
- PSA Birth Certificate
- Valid passport or valid government issued ID* of adoptive parent/s ( click here for the List of Acceptable IDs for Passport Application )
- Court Decree of Adoption
- Applicant is not accompanied by adoptive parent/s during the application process, applicant must submit a Notarized Special Power of Attorney executed by the adoptive parent/s designating the minor’s companion to assist in the application process. The Special Power of Attorney must be authenticated by the Philippine Consulate if executed in Australia.
- If the applicant is not traveling with adoptive parent/s, the applicant must submit DSWD clearance and Affidavit of Support and Consent executed by the adoptive parent/s. The Affidavit of Support and Consent must be authenticated by the Philippine Embassy/Consulate if executed abroad.
- Personal Appearance of minor applicant and ICAB representative
