PASSPORT REQUIREMENTS FOR ADULT - NEW APPLICATION

In order to facilitate the smooth processing of passport applications (New, Renew & Lost), all applicants are requested to book an online appointment using the link below and fill-out the attached fillable Passport Application Form on the email confirmation for submission during application together with other requirements.
The Philippine Consulate requests that the application be typewritten for easy reference during data encoding.
Please write / reflect the following in the Passport Application Form, if not yet written:
- Complete Address including Suburb/Town/Region and Postal Code;
- Complete Name of Wife / Husband in this format: FIRST NAME MIDDLE NAME LAST NAME (Example: JUAN SANTOS CRUZ);
- Complete Maiden / Single Name of the Mother in this format: FIRST NAME MIDDLE NAME LAST NAME (Example: MARIA DEL ROSARIO SANTOS);

- PERSONAL APPEARANCE of the applicant
- Printed BARCODED APPLICATION FORM, which can be found on the confirmation email after making an appointment;
- ORIGINAL AND PHOTOCOPY of PSA Birth Certificate or PSA Report of Birth
- If Report of Birth was registered one (1) year or more from date of passport application, bring original and photocopy of the Philippine Statistics Authority (PSA) (formerly National Statistics Office of NSO) Report of Birth ( Apply here for PSA Birth certificates )
- For Married Female Applicants – ORIGINAL AND PHOTOCOPY of PSA Marriage Certificate or PSA Report of Marriage
- Photocopy of valid Government-issued ID (click here for the List of Acceptable IDs for Passport Application ).
- Valid Australian Visa – Photocopy;
- Passport Fee: A$108.00 Cash and Debit Card Only
- “Express Post” envelope with Signature on Delivery sticker, which can be obtained at any Australia Post.
- Single Passport Applicant – Medium-Sized Envelope
- Family Passport Applicant (Maximum of 4 Passport Applicants) – Large Sized Envelope

Please provide PHOTOCOPIES of the documents below, as applicable:
- Applicant is a DUAL CITIZEN
- ORIGINAL and PHOTOCOPY of PSA Birth Certificate.
- For Married Female Applicants – ORIGINAL AND PHOTOCOPY of PSA Marriage Certificate or PSA Report of Marriage.
- ORIGINAL and PHOTOCOPY of Identification Certificate, Oath of Allegiance and Order issued by the Philippine Embassy / Consulate or the Bureau of Immigration (BI).
- ORIGINAL and PHOTOCOPY of latest Philippine Passport.
- Government issued IDs (from the Philippines or from country of second citizenship e.g. Australian Passport or Driver’s License).
- Late-Registered Birth Certificate
- If Birth Certificate was registered at least ten (10) years ago, application will be treated as a regular application, and no additional supporting documents will be required.
- If Birth Certificate was registered less than ten (10) years ago, applicant must submit IDs that pre-date the late registration.
- If applicant has no ID’s that pre-date the late registered Birth certificate, applicant must produce current IDs and National Bureau of Investigation (NBI) Clearance.
- NO Birth Certificate or Report of Birth
- If applicant was born AFTER 1950 – applicant must first file for late registration with Local Civil Registrar (LCR) or Consular Office with jurisdiction over the place where applicant was born. Applicant will then submit the original copy of the late registered PSA Birth Certificate with the requisite supporting documents and IDs that pre-date the late registration.
- If applicant was born IN OR BEFORE 1950 – applicant must submit original copies of PSA Certificate of No Birth Record and Affidavit of Two Disinterested Persons attesting to the identity of the applicant.
- Female applicant’s marriage has been dissolved and she wants to revert to her maiden surname
- Applicant must submit original copy of the PSA Marriage Certificate (MC) with an annotation stating that the marriage has been dissolved.
- Applicant has lacking data in Birth Certificate or Report of Birth
- Applicant must submit proof of filing of supplemental report or correction from the Local Civil Registrar (LCR)
- Applicant has discrepancy in data in their Birth Certificate / Report of Birth and other documents
- If the discrepant data is in the BC, the applicant must submit the original PSA annotated birth certificate reflecting the correct entry. If the discrepant data is in the other documents, the BC will be followed.
- Applicant is a Naturalized Filipino Citizen
- Certificate of Naturalization from BI
- Applicant obtained Filipino citizenship by Election
- Affidavit of Election of Philippine Citizenship
- Certificate of Election from BI
- Applicant has been granted citizenship by Legislation
- Certified true copy of the law granting citizenship
- Original Apostillized foreign Birth Certificate
