CITIZEN’S CHARTER

3.1 REPORT OF BIRTH

The birth of a child born to any Filipino parent in Victoria, South Australia and Tasmania, Australia must be reported to the Philippine Consulate General in Melbourne within one (1) year after the date of birth. If reported after one (1) year, the registration is deemed delayed. The birth may nevertheless be recorded upon the determination of the consular officer of satisfactory evidence on the authenticity of the report. In these cases, the person who executed the report shall furnish the Consulate with an explanation surrounding the delay in reporting the birth.

• Fills out Order of Payment form and hands it to the client, together with pertinent documents, for cash / debit card payment to the Cashier;

• Requests the client to proceed to Window 1 (Cashier) for cash / debit card payment.

Note:  Applicants are encouraged to email to [email protected] their duly filled-out Report of Birth form for checking before booking an appointment.

1.1 If filed in person - Four (4) accomplished in typewritten form to be  

       signed in front of the Consular Officer;

1.2 If sent by mail - Four (4) original copies accomplished in typewritten form must be notarized in item no. 20 with original signature in each copy by Philippine Consul, Notary Public or Justice of Peace.

2)  Original Birth Certificate of the child with registry number, issued by the             Civil Registrar or Registry of Births, Death and Marriages of the state/territory where the birth was registered plus four (4) photocopies.

(Note: Original documents will be returned together with the applicant’s copy of the Report of Birth of your child).

3) If parents are married:

3.1 If married in the Philippines, original PSA issued Marriage Certificate or original Australian Marriage Certificate with registry number of parents plus four (4) photocopies. (Note: Original copy will be returned           together with the applicant’s copy of the processed Report of Birth of           your child); 

          or

         

four (4) certified true copies of the Philippine marriage certificate issued by the Philippine Statistics Authority (PSA).  To apply online, please visit https://psaserbilis.com.ph/Census/MarriageCertificate

3.2 If married in Australia, four (4) certified true copies of Marriage Certificate with registration number issued by the Civil Registry and four (4) certified true copies of Report of Marriage issued by the Philippine Embassy/Consulate.

4) Four (4) certified true copies each of the data page of valid passports of parents or other form of identification, i.e. driver’s license if valid passport copy is not available.

5) Four (4) certified true copies each of the parent’s proof of citizenship status (e.g., Australia visa, Dual Citizenship documents).

• If sending by mail: Postal money order of AU$45.00 payable to the Philippine Consulate General in Melbourne and a self-addressed stamped mailing envelope with Registered/Express Post envelope and signature on delivery sticker.

ADDITIONAL REQUIREMENTS:

6) For late registration

6.1 If reporting at least 1 year after the child’s birth, an Affidavit of Late    Registration of Birth is required (Downloaded from Melbourne PCG website);

6.2 If reporting 5 years or more after the date of birth, a recently-issued original PSA Certificate of No Report of Birth record is required.

7) For legitimated child (if parents, who did not have any legal    impediment to marry each other at the time of the child’s birth, were    married subsequently).

7.1 Joint Affidavit of Legitimation executed and signed by both parents and duly notarized by Australian Notary Public or Justice of the Peace - one (1) original and three photocopies. (Downloaded from Melbourne PCG website).

8) For illegitimate child using mother’s maiden surname

8.1 One (1) original PSA authenticated birth certificate of mother, plus 4

photocopies. (Note: Original documents will be returned together with the applicant’s copy of the Report of Birth of your child).

9) If Illegitimate and Using Father’s Surname

9.1 Four (4) original Affidavit of Admission of Paternity (AAP) duly executed and signed by the child’s father before a consular officer at the Consulate, if filed in person. (Downloaded from Melbourne PCG website)

9.2 Copies of father’s valid passport and the unregistered birth certificate       of the child are required.
 
If this document was executed outside Australia, it must be notarized and registered in the Philippine Embassy or Consulate in the country where it was executed.

If executed in the Philippines, it must be duly notarized and  registered with the relevant local civil registrar.

9.3 Four (4) original of Affidavit to Use Surname of Father (AUSF),                       executed by the child’s mother before a consular officer at the Consulate, if filed in person.  (Downloaded from Melbourne PCG website)

9.4 Four (4) copies each of mother’s valid passport and the unregistered    

birth certificate of the child.
 

If this document was executed outside Australia, it must be notarized and registered in the Philippine Embassy or Consulate in the country where it was executed.

If executed in the Philippines,it must be duly notarized and  registered with the relevant Local Civil Registrar.

 

9.5 Certificate of Registration - The Consulate will issue the Certificate of Registration upon submission of the Affidavit of Admission of Paternity (AAP) and the Affidavit to Use Surname of the Father (AUSF).

10) If the Filipino acquired Australian citizenship before the birth:

10.1 Four (4) certified true copies of the parents’ Australian Certificate of Citizenship;

10.2 Four (4) certified true copies of the parents’ old Philippine passport.

11) If the Filipino acquired Australian citizenship after the birth:

11.1 If the Filipino parent reacquired/retained his/her Philippine citizenship after the birth of the child:

              11.1.1 Four (4) certified true copies of the documents pertaining to

                         the re-acquisition/retention of his/her Philippine citizenship.

Note: The Consulate reserves the right to require additional documents as deemed necessary.

4) Original and four (4) certified true copies of the data page of valid passports

of both parties or other form of valid identification, i.e., driver's license, if

valid passport copy is not available.

5) Proof of Status of Philippine Citizenship of the Applicant (Visa issued by

Australian, Dual Citizenship documents)

If sending by mail: Postal money order of AU$45.00 payable to the Philippine Consulate General in Melbourne and a self-addressed stamped mailing envelope with Registered/Express Post envelope and signature on delivery sticker.

ADDITIONAL REQUIREMENTS:

7) For the Filipino spouse with previous marriage/s

7.1 If applicant was previously married but now is DIVORCED:

7.2  If applicant was previously married but now is ANNULLED:

6) Return registered envelope i.e. Express Post envelope with signature on delivery sticker for the return of ROD.

7) Requirements for Consular Mortuary Certificate

    Original and two (2) photocopies of the following:

7.1  Death Certificate issued by the Australian Register of Birth, Death and Marriages;

7.2  Embalming Certificate, if the remains are embalmed;

7.3  Cremation Certificate, if the remains are cremated;

7.4  Travel itineraries for the remains and the one who will accompany the remains;

7.5  Passport of the deceased;

7.6  Passport of the one who will accompany the remains;

7.7  Medical Certificate of Death;

7.8  Burial or Transit Permit for shipment of the human remains;

7.9  Mortuary Certificate stating that the casket or the urn contains only the body or the cremains of the deceased;

7.10 Processing fee of AU$45.00 payable via debit card or cash or if by mail, postal money order made payable to the Philippine Consulate General in Melbourne. (Note: Do not include cash payment in the mailing envelope)

7.11 Return registered envelope i.e. Express Post envelope with signature on delivery sticker for the return of ROD and/or Consular Mortuary Certificate
