PASSPORT REQUIREMENTS FOR ADULT - RENEWAL APPLICATION
Brown Passports, Green Passports, Machine Readable Passports (issued from 2007 to 2010) or any older Passports will be treated as NEW APPLICATION. For Requirements for New Adult Applicants, read here.
ADVISORY
In order to facilitate the smooth processing of passport applications (New, Renew & Lost), all applicants are requested to book an online appointment using the link below and fill-out the attached fillable Passport Application Form on the email confirmation for submission during application together with other requirements.
The Philippine Consulate requests that the application be typewritten for easy reference during data encoding.
Please write / reflect the following in the Passport Application Form, if not yet written:
- Complete Address including Suburb/Town/Region and Postal Code;
- Complete Name of Wife / Husband in this format: FIRST NAME MIDDLE NAME LAST NAME (Example: JUAN SANTOS CRUZ);
- Complete Maiden / Single Name of the Mother in this format: FIRST NAME MIDDLE NAME LAST NAME (Example: MARIA DEL ROSARIO SANTOS);

- Personal Appearance
- Printed BARCODED APPLICATION FORM, which can be found on the confirmation email after making an appointment;
- Latest Philippine ePassport – Original;
- Latest Philippine ePassport – Photocopy of the data page;
- Valid Australian Visa – Photocopy;
- Passport Fee: A$108.00 Cash and Debit Card Only (Please bring the physical card)
- “Express Post” envelope with Signature on Delivery sticker, which can be obtained at any Australia Post.
- Single Passport Applicant – Medium Sized Envelope
- Family Passport Applicant (Maximum of 4 Passport Applicants) – Large Sized Envelope
SIGNATURE ON DELIVERY STICKER
- For Change of Name (in addition with the Renewal Core Requirements)
Original and Photocopy of the PSA Birth Certificate and any of the following that will support the change of name (if applicable):- Original PSA Marriage Certificate and one (1) photocopy;
- Original PSA Annotated Marriage Contract showing annulment/ divorce/ court ordered instruction and one (1) photocopy;
- Original PSA Death Certificate of Spouse (if spouse is Filipino) of Original Apostilled Death Certificate (if spouse is not Filipino).
- For Dual Citizen (in addition with the Renewal Core Requirements)
- Original and Photocopy of Identification Certificate, Order and Oath Allegiance issued by the Philippine Embassy/Consulate General or the Bureau of Immigration
- For Naturalized Citizen (in addition with the Renewal Core Requirements)
- Identification Certificate of Naturalization from Bureau of Immigration
- For Filipino citizen by Election (in addition with the Renewal Core Requirements),
- Affidavit of Election of Philippine Citizenship
- Identification Certificate of Election from BI
