  • Lost passport applications will be treated as NEW APPLICATION.
  • There will be 15 calendar days before the passport is processed.
  • Total fee for Lost Passport is A$315.00 Cash and Debit Card Only
    (Affidavit of Loss – A$45.00 and Replacement for Lost Passport – A$270.00)
LOST PASSPORT IS STILL VALID
  • Affidavit of Loss in English

  • Original and Photocopy of Police Report in English
LOST PASSPORT IS EXPIRED
  • Affidavit of Loss in English

MUTILATED OR DAMAGED PASSPORT
  • Treated as NEW APPLICATION
  • Affidavit of Explanation
    • Affidavit of Explanation – A$45.00 Cash and Debit Card Only
  • Applicant must surrender mutilated / damaged passport
  • PERSONAL APPEARANCE of the applicant
  • Printed BARCODED APPLICATION FORM, which can be found on the confirmation email after making an appointment;
  • ORIGINAL AND PHOTOCOPY of PSA Birth Certificate or PSA Report of Birth
  • For Married Female Applicants – ORIGINAL AND PHOTOCOPY of PSA Marriage Certificate or PSA Report of Marriage
  • Photocopy of valid Government-issued ID ( click here for the List of Acceptable IDs for Passport Application )
  • Australian Visa – Photocopy;
  • Lost Passport Fee: A$270.00 Cash and Debit Card Only
  • “Express Post” envelope with Signature on Delivery sticker, which can be obtained at any Australia Post.
    • Single Passport Applicant – Medium-Sized Envelope
    • Family Passport Applicant (Maximum of 4 Passport Applicants) – Large-Sized Envelope

In case applicant is a Dual Citizen,

  • Government Issued IDs (Refer to Valid Philippine Government Issued IDs or their secondary citizenship/ residence counterparts)
  • Original and Photocopy of Order, Oath of Allegiance and Identification Certificate issued by the Philippine consulate or the Bureau of Immigration

In case applicant is a Naturalized Citizen,

  • Original and Photocopy of Identification Certificate of Naturalization from Bureau of Immigration

In case applicant is a Filipino citizen by Election,

  • Affidavit of Election of Philippine Citizenship
  • Original and Photocopy Identification Certificate of Election from BI
