CONSULAR MORTUARY CERTIFICATE
- If the deceased person was still a Filipino at the time of his death, Report of Death may be applied at the Philippine Consulate. Please see this link: Link
- The Report of Death will be sent to the Philippine Statistics Authority and will be considered the registered Death Certificate of the deceased.
- Consular Mortuary Certificate is a document needed for the shipment of human remains from Australia to the Philippines.
- Consular Mortuary Certificate will be released after three (3) working days.
- All the original documents will be returned together with the official document to be issued by the Consulate

- Report of Death
- Original and two (2) Photocopies
- Death Certificate issued by the Australian Registrar of Births, Deaths and Marriages;
- It must indicate the personal circumstances of the deceased;
- Apostilled by the Australian Department of Foreign Affairs and Trade (DFAT)
- Original and Two (2) Certified true copies
- Original Mortuary/Funeral Director’s Certificate, stating that:
- The body was properly embalmed/cremated in accordance with internationally-accepted health practices;
- It is in a sanitary condition consistent with existing standards for overseas shipment; and
- The casket/urn has been hermetically sealed and contains only the remains of the deceased
- Original and Certified true copy
- Original Certificate of Non-Contagious Disease issued by the Local Area Health Service, stating that
- The deceased did not die of a contagious disease or
- The remains to be shipped did not originate from a place contaminated by a contagious disease.
- Original and two (2) Certified true copies
- Original Transit Permit issued by the Local Area Health Service at the place where the transportation begins.
- Proof of Identity of the Deceased (if non-Filipino):
- Photocopy of the Australian passport.
- Proof of Filipino citizenship of the deceased, if applicable, Certified true copies of the deceased’s:
- Philippine passport; or
- Australian Permanent Resident Card or Work Permit, or
- Dual Citizenship documents (Identification Certificate and Approval Order).
- Name and address of consignee, name of carrier, date and port of embarkation, port of destination and scheduled time of arrival.

- If filed in person: AUD45.00 Cash and Debit Card only (Please bring the physical card)
- If sent by mail: AUD45.00 Postal Money Order or Bank Cheque payable to the PHILIPPINE CONSULATE GENERAL IN MELBOURNE
- Include an “Express Post” large-sized (A4) envelope which can be obtained at any Australia Post for return mail
- the processed Consular Mortuary Certificate will be sent to an Australian address using this envelope
- the Consulate assumes no responsibility for any loss or damage in the mail or courier service
- with Signature on Delivery sticker

- Certification from the Funeral parlor stating that the urn contains only the ashes of the deceased person
- Name of consignee and address where the urn will be brought in the Philippines
- Present address of the Person who will bring the urn to the Philippines
- Passport data page – Four (4) certified true copies
