CONSULAR MORTUARY CERTIFICATE

  • If the deceased person was still a Filipino at the time of his death, Report of Death may be applied at the Philippine Consulate. Please see this link: Link 
  • The Report of Death will be sent to the Philippine Statistics Authority and will be considered the registered Death Certificate of the deceased.
  • Consular Mortuary Certificate is a document needed for the shipment of human remains from Australia to the Philippines.
  • Consular Mortuary Certificate will be released after three (3) working days.
  • All the original documents will be returned together with the official document to be issued by the Consulate
  1. Duly accomplished and signed REQUEST FORM for Consular Mortuary Certificate applications [Click here to download the form] .
  2. Report of Death
    1. Original and two (2) Photocopies
  3. Death Certificate issued by the Australian Registrar of Births, Deaths and Marriages;
    1. It must indicate the personal circumstances of the deceased;
    2. Apostilled by the Australian Department of Foreign Affairs and Trade (DFAT)
    3. Original and Two (2) Certified true copies
  4. Original Mortuary/Funeral Director’s Certificate, stating that:
    1. The body was properly embalmed/cremated in accordance with internationally-accepted health practices;
    2. It is in a sanitary condition consistent with existing standards for overseas shipment; and
    3. The casket/urn has been hermetically sealed and contains only the remains of the deceased
    4. Original and Certified true copy
  5. Original Certificate of Non-Contagious Disease issued by the Local Area Health Service, stating that
    1. The deceased did not die of a contagious disease or
    2. The remains to be shipped did not originate from a place contaminated by a contagious disease.
    3. Original and two (2) Certified true copies
  6. Original Transit Permit issued by the Local Area Health Service at the place where the transportation begins.
  7. Proof of Identity of the Deceased (if non-Filipino):
    1. Photocopy of the Australian passport.
  8. Proof of Filipino citizenship of the deceased, if applicable, Certified true copies of the deceased’s:
    1. Philippine passport; or
    2. Australian Permanent Resident Card or Work Permit, or
    3. Dual Citizenship documents (Identification Certificate and Approval Order).
  9. Name and address of consignee, name of carrier, date and port of embarkation, port of destination and scheduled time of arrival.
  10. Travel itinerary / flight details
  11. Fee:
    1. If filed in person: AUD45.00 Cash and Debit Card only
    2. If sent by mail: AUD45.00 Postal Money Order or Bank Cheque payable to the PHILIPPINE CONSULATE GENERAL IN MELBOURNE
  12. Include an “Express Post” large-sized (A4) envelope which can be obtained at any Australia Post for return mail
    1. the processed Consular Mortuary Certificate will be sent to an Australian address using this envelope
    2. the Consulate assumes no responsibility for any loss or damage in the mail or courier service
    3. with Signature on Delivery sticker

  1. Duly accomplished and signed request form for a Certificate to transport cremated remains [Click here to download the form] .
  2. Report of Death
    1. Original and Photocopy
  3. Death Certificate issued by the Australian Registrar of Births, Deaths and Marriages;
    1. It must state the personal circumstances of the deceased;
    2. Apostilled by the Australian Department of Foreign Affairs and Trade (DFAT)
    3. Original and Certified true copy
  4. Proof of Identity of the Deceased (if non-Filipino):
    1. Photocopy of the Australian passport.
  5. Proof of Filipino citizenship of the deceased, if applicable, Certified true copies of the deceased’s:
    1. Philippine passport; or

    4. One (1) Certified true copy
  6. Certificate of Cremation (for cremated remains)
    1. Original and Certified true copy
  7. Certification from the Funeral parlor stating that the urn contains only the ashes of the deceased person
  8. Name of consignee and address where the urn will be brought in the Philippines
  9. Present address of the Person who will bring the urn to the Philippines
    1. Passport data page – Four (4) certified true copies
    2. Flight details
  10. Fee:
    1. If filed in person: AUD45.00 Cash and Debit Card only
    2. If sent by mail: AUD45.00 Postal Money Order or Bank Cheque payable to the PHILIPPINE CONSULATE GENERAL IN MELBOURNE
