REPORT OF BIRTH (ROB)

The Report of Birth is a declaration by the person furnishing information on the birth abroad of a Filipino child. It is important for Filipino parents of children born abroad to promptly register with the Consulate the birth of their child in order for such a birth to be registered with the Philippine Statistics Authority (PSA).

When a child is born abroad to a Filipino parent/parents, i.e. one who has not been naturalized as a citizen of a foreign country, the child’s birth must be immediately reported to the Philippine Embassy or Consulate General which exercises jurisdiction over the place of birth. The Report of Birth may be accepted as proof of Philippine citizenship of a child applying for a Philippine passport, if such ROB has not been transmitted to PSA for more than six (6) months prior to passport application.

If the ROB had been transmitted for more than six (6) months, then PSA birth certificate shall be required.

Birth of a Filipino citizen in Victoria, South Australia and Tasmania must be reported to the Philippine Consulate General in Melbourne.

  • Kindly download, use and print the Report of Birth (ROB) form of the Philippine Consulate General in Melbourne

  • Save in your email a soft copy of the accomplished ROB form for easy editing should there be a need to correct entries to comply with the requirements and policies of the Philippines;
  • The Consulate highly recommends that the form be typewritten with no erasures as this is considered as a permanent record;
  • The Consulate has the discretion to reject forms that are:
    • Sloppily accomplished;
    • With erasures;
    • With wrong entries; or
    • With a wrong stamp by the notary public.
  • Original documents submitted will be returned to the applicant together with the Applicants’ Copy of the Report of Birth of your child;
  • Filing of Report of Birth can be done either IN PERSON (which requires appointment) or BY MAIL;
  • Report of Birth will be released after THREE (3) WORKING DAYS;
  • PASSPORT APPLICATION may only be done on the date of release of the report of birth. Appointment is also required for passport application. Please see link below:

  • Please encode the requested information in the PDF form
    • Print in four (4) copies
  • For item number 3: Child’s middle name should be the mother’s single surname
  • For item number 4: Place of birth of the child indicate the city/state/country as indicated in the Birth Certificate
    • Example: Melbourne, Victoria, Australia
  • For item numbers 9-11: Mother (maiden information) should be the mother’s name when she is still single

  1. Duly accomplished and signed REPORT OF BIRTH FORM

    • If filed in person  – Four (4) Original accomplished Report of Birth forms
      • To be signed in front of the Consular Officer in the Philippine Consulate in Melbourne
    • If sent by mail – Four (4) Original accomplished Report of Birth forms with original signature of the parent and original signature of the NOTARY PUBLIC in EVERY PAGE of the Report of Birth
  2. FOREIGN BIRTH CERTIFICATE of the Child
    • With a registry number issued by the Civil Registrar or Registry of Births, Deaths and Marriages of the state/territory where the birth took place in Australia
    • Original and four (4) photocopies
  3. Data page of VALID PASSPORTS OF PARENTS or other form of identification, i.e. driver’s license, if valid passport copy is not available
    • Original and four (4) photocopies
  4. Original PSA-issued MARRIAGE CERTIFICATE (or original Australian Marriage Certificate with registry number) of parents
    • Original and four (4) photocopies
    • If parents are not married, please see additional requirements below
  5. Proof of Parent’s Status such as:
    • Australian Visa; or Dual Citizenship documents (i.e. Identification Certificate, Order and Oath of Allegiance)
    • Original and Four (4) Photocopies
  6. Fee:
    • If filed in person: AUD45.00 Cash and Debit Card Only
    • If sent by mail: AUD45.00 Postal Money Order or Bank Cheque payable to the PHILIPPINE CONSULATE GENERAL IN MELBOURNE
  7. For Applicant’s by mail – Include an “Express Post” large-sized (A4) envelope which can be obtained at any Australia Post for return mail
    • The processed ROM will be sent to an Australian address using this envelope
    • The Consulate assumes no responsibility for any loss or damage in the mail or courier service

  1. If reporting ONE (1) YEAR or more after the child’s birth:
    1. Affidavit of Delayed Registration
      • If filed at the Consulate – To be signed by the Affiant in front of the Consular Officer
      • Four (4) Original Copies
      • Fee: AUD45.00 Cash and Debit Card Only

      If application by mail – executed and signed by the affiant and duly notarized by Australian notary public/lawyer. Submit one (1) original and three photocopies.

  2. If reporting more than FIVE (5) YEARS after the child’s birth:
    1. Affidavit of Delayed Registration
      • If filed at the Consulate – To be signed by the Affiant in front of the Consular Officer
      • Four (4) Original Copies
      • Fee: AUD45.00 Cash and Debit Card Only

      If application by mail – executed and signed by the affiant and duly notarized by Australian notary public/lawyer. Submit one (1) original and three photocopies.

    2. Certificate of No Record of Birth from the Philippine Statistics Authority (PSA)
  1. If Not Married: Using the Last Name of the Father:

      • If filed at the Consulate – Executed by the Mother

    2. Affidavit of Admission of Paternity (AAP)
      • If filed at the Consulate – Executed by the Father
      • Four (4) Original Copies
      • To be signed by the Affiant in front of the Consular Officer

      • If application by mail – executed and signed by the father and duly notarized by Australian notary public/lawyer. Submit one (1) original and three (3) photocopies.
      • Copies of father’s valid passport and the unregistered or registered birth certificate of the child
      • Copies of mother’s valid passport and the unregistered or registered birth certificate of the child
    3. CERTIFICATION based on AUSF and APP will be prepared by the Consulate

    2. One (1) original PSA authenticated birth certificate of mother plus three (3) photocopies.
  3. If Parents Subsequently Married after Birth (Legitimated)

    1. Joint Affidavit of Legitimation by Both Parents
      • If filed at the Consulate – To be signed by the Affiants in front of the Consular Officer
      • Four (4) Original Copies

      • If application by mail – executed and signed by both parents and duly notarized by Australian notary public/lawyer. Submit one (1) original and three (3) photocopies
  1. If the Filipino parent acquired Australian citizenship BEFORE the birth of the child:
    • Four (4) photocopies of the parents’ Australian Certificate of Citizenship
    • Four (4) photocopies of the parents’ old Philippine passport
  2. If the Filipino parent reacquired/retained his/her Philippine citizenship AFTER the birth of the child:
    • Four (4) photocopies of Dual Citizenship documents of the parents (i.e. Identification Certificate, Order and Oath of Allegiance)
