The Report of Birth is a declaration by the person furnishing information on the birth abroad of a Filipino child. It is important for Filipino parents of children born abroad to promptly register with the Consulate the birth of their child in order for such a birth to be registered with the Philippine Statistics Authority (PSA).
When a child is born abroad to a Filipino parent/parents, i.e. one who has not been naturalized as a citizen of a foreign country, the child’s birth must be immediately reported to the Philippine Embassy or Consulate General which exercises jurisdiction over the place of birth. The Report of Birth may be accepted as proof of Philippine citizenship of a child applying for a Philippine passport, if such ROB has not been transmitted to PSA for more than six (6) months prior to passport application.
If the ROB had been transmitted for more than six (6) months, then PSA birth certificate shall be required.
REQUIREMENTS
- Duly accomplished and signed Report of Birth form.

- If sent by mail, 1 original copy notarized by notary public plus three (3) Certified True Copies(CTC).

Note: The Consulate highly recommends that the form be typewritten. No erasures please as this will be the permanent record. The Consulate has the discretion to reject forms that are sloppily accomplished or with erasures or with wrong entries and with wrong stamp by the notary public.
- Original Birth Certificate of the child with registry number issued by the Civil Registrar or Registry of Births, Deaths and Marriages of the state/territory where the birth was registered plus four(4) photocopies. (Note: Original will be returned together with the applicants’ copy of the Report of Birth of your child.)
- Original PSA-issued Marriage Certificate (or original Australian Marriage Certificate with registry number) of parents plus four(4) photocopies. (Note: Original will be returned together with the applicants’ copy of the Report of Birth of your child.)
- Four (4) certified true copies of the data page of valid passports of parents or other form of identification, i.e. driver’s license if valid passport copy is not available.
- Four (4) certified true copies of the parent’s proof of status (Visa issued by Australia, Dual Citizenship documents)
ADDITIONAL REQUIREMENTS
For late registration

- If reporting at least 1 year after the child’s birth, an Affidavit of Late Registration of Birth is required;

- If reporting 5 years or more after the date of birth, a recently-issued original PSA certificate of no report of birth record is required.
If legitimated (if parents, who did not have any legal impediment to marry each other at the time of the child’s birth, were married subsequently)

- Joint Affidavit of Legitimation executed and signed by both parents and duly notarized by Australian notary public/lawyer.
Submit one (1) original and three photocopies.

If illegitimate and Using Mother’s Maiden Surname

- Affidavit of Illegitimacy duly executed and signed by mother and duly notarized by Australian notary public/lawyer.
Submit one (1) original and three (3) photocopies; and
- Affidavit of Illegitimacy duly executed and signed by mother and duly notarized by Australian notary public/lawyer.

- One (1) original PSA authenticated birth certificate of mother plus 3 photocopies. (Note: Original will be returned together with the official copy of the Report of Birth of your child.)
If Illegitimate and Using Father’s Surname

- One (1) original Affidavit of Admission of Paternity (AAP) plus three (3) photocopies duly executed by the child’s father, to be signed before a consular officer at the Consulate, if filed in person.

- Copies of father’s valid passport and the unregistered birth certificate of the child are required.
If this document was executed outside Australia, it must be notarized and registered in the Philippine Embassy or Consulate in the country where it was executed; if executed in the Philippines, by a notary public and it will be registered by the local civil registrar where the birth of the child is to be registered.

- One (1) original of Affidavit to Use Surname of Father (AUSF) plus three (3) photocopies, executed by the child’s mother, to be signed before a consular officer at the Consulate, if filed in person.

- Copies of mother’s valid passport and the unregistered birth certificate of the child are required.
If this document was executed outside Australia, it must be notarized and registered in the Philippine Embassy or Consulate in the country where it was executed; if executed in the Philippines, by a notary public and it will be registered by the local civil registrar where the birth of the child is to be registered.
- Certificate of Registration
The Consulate will issue the Certificate of Registration upon submission of the Affidavit of Admission of Paternity (AAP) and the Affidavit to Use Surname of the Father (AUSF)
If the Filipino acquired Australian citizenship after the birth of the child:
- Four (4) certified true copies of the parents’ Australian Certificate of Citizenship
- Four (4) certified true copies of the parents’ old Philippine passport
If the Filipino parent reacquired/retained his/her Philippine citizenship after the birth of the child:
- Four (4) certified true copies of the documents pertaining to the re-acquisition/retention of his/her Philippine citizenship.
Note: Additional documents may be required by the Consular Officer.

Send all the requirements and applicable additional documents along with the following:
- A$ 45 payment for the Report of Birth
- A$ 45 payment for Affidavit of Delayed Registration of Birth, if applicable
- A$ 45 payment for Affidavit of Admission of Paternity (AAP), if applicable
- A$ 45 payment for Affidavit to Use Surname of the Father (AUSF), if applicable
- A$ 45 payment for the Certificate of Registration to be issued by the Consulate for AAP and AUSF, if applicable
- Return Registered envelope, person to person (blue sticker) or Express Post envelope, signature on delivery (large size)

The Consulate only accepts payments in:
- CASH (In person filing)
- POSTAL MONEY ORDER or
- BANK CHEQUE
