GUIDELINES ON THE REPATRIATION OF HUMAN REMAINS TO THE PHILIPPINES AND REQUIREMENTS FOR BRINGING OF URN CONTAINING ASHES OF DECEASED PERSON TO THE PHILIPPINES

Guidelines on the repatriation of human remains to the Philippines

In instances where the embalmed remains of a deceased person are to be shipped to the Philippines, the Philippine Consulate shall issue a Consular Mortuary Certificate upon submission of the following documentation:

  1. A Death Certificate issued by the State Registry showing the personal circumstances of the deceased
  2. A Mortuary/Funeral Director’s Certificate duly authenticated by the Australia Department of Foreign Affairs and Trade (DFAT), stating that the body was properly embalmed/cremated in accordance with internationally-accepted health practices; that it is in a sanitary condition consistent with existing standards for overseas shipment; and that the casket/urn has been hermetically sealed and contains only the remains of the deceased
  3. A Certificate of Non-Contagious Disease issued by the Local Area Health Service, stating that the deceased did not die of a contagious disease or that the remains to be shipped did not originate from a place contaminated by a contagious disease
  4. A Transit Permit issued by the Local Area Health Service at the place where the transportation begins
  5. Information on the consignee/addressee/person, name of carrier, date and port of embarkation, port of destination and scheduled time of arrival
  6. For deceased Filipino citizens/ passport holders, a Report of Death issued by the Philippine Embassy/Consulate. The fee for the processing of the report of death is A$45, please bring cash or by postal money order payable to the Philippine Consulate General.
  7. Copy of the passport of the deceased Filipino national
SUBMISSION OF REQUIREMENTS

In person at the Philippine Consulate at Level 10 257 Collins St., Melbourne VIC 3000

If the report is submitted to the Consulate through mail, the form should be subscribed and sworn to by a Justice of Peace or the Notary Public under item number 26 of the Report of Death. A return Registered envelope, person to person (blue sticker) or Express Post envelope, signature on delivery is needed. The Consulate assumes no responsibility for any loss or damage through the mail or courier service.

Requirements for bringing of urn containing ashes of deceased person to the Philippines
  1. Certification to be issued by the Consulate
  2. Report of Death (if the deceased is a Filipino national)
  3. Death Certificate
  4. Certificate of Cremation
  5. Copy of the passport of the deceased
  6. Copy of the passport of the person who will bring the urn to the Philippines
  7. Flight details, if any
  8. Fee of A$45 (Cash) for the certification and A$45 (Cash) for the Report of Death also payable by postal money order payable to the Philippine Consulate General.
