1. Letter from employer or manning agency stating the vessel where the applicant will work and the Philippine port where the applicant will board the vessel. (Seamen are allowed to present a one-way plane ticket.)
2. Notarized and duly accomplishednon-immigrant visa application (please see attached form)
3. Electronic or machine-readable passport valid for at least six (6) months beyond authorized period of stay in the Philippines.
4. One (1) recently taken colored photo 1.77″ x 1.37″ (3.5 cm x 4.5 cm), front view, with a white background; no eyeglasses or sleeveless attire. Digital photographs are not allowed.
5. Payment of visa fee in cash or Australian Money Order payable to the Philippine Consulate General Melbourne.
6. A self-addressed pre-paid stamped envelope, with tracking number, must be included when sending the application.

Visa (Seaman Visa) fees:

a. Individual	AUD 36.00
b. Crew list visa of vessels with: Not more than forty (40) crew members	AUD 180.00
c. Crew list visa of vessels with: From 41-100 crew members	AUD 270.00
d. Crew list visa of vessels with: From 101-200 crew members	AUD 360.00
e. Crew list visa of vessels with: More than 200 crew members	AUD 450.00
